member complete Section C then have member sign the form and return to our office

Tax Certification Letter
When a member terminates employment or retires and is eligible to cash out their pension
assets or chooses to start a retirement income under a product offered by Sun Life Group
Retirement Services, the Tax Exemption Certification letter will need to be completed and

Non-Registered Savings Plan (NREG) Enrolment Form
The new plan member must complete this form before the date of enrolment in the

Appointment of Trustee or Administrator for a Minor Beneficiary
The Plan member must complete this form if a minor beneficiary is assigned under

Request to Withdraw Funds
The Plan Sponsor & Plan Member must complete & sign this form upon termination of
employment or retirement, and in order to request a withdrawal of or transfer funds available.
